
The Adult Disability Report is one of the 3 main forms you must submit when applying for Social Security disability benefits. On this form you are required to complete 10 sections of information plus one optional section for SSI recipients.
The 11 sections of the Adult Disability Report are:
The first 2 sections, Information and Contacts, gather the basic contact information for the disabled person and the person helping them complete the application (if any) or another person who could answer questions related to their disability status.
The Medical Conditions, Medicines, Medical Treatment, and Other Medical Information sections all gather extensive information about treatment history, testing, medications, and treating/prescribing medical professionals. These sections help identify the disabling condition and form a timeframe for the disability duration and severity.
The Work Activity, Education and Training and Job History sections serve to prove that the claimant worked enough years to qualify for Social Security disability benefits. It also serves to prove that they are no longer able to work because of their disability and the nature of their education, training and former employment.
