What’s asked on the Adult Disability Report?

The Adult Disability Report is one of the 3 main forms you must submit when applying for Social Security disability benefits. On this form you are required to complete 10 sections of information plus one optional section for SSI recipients.

The 11 sections of the Adult Disability Report are:

  • Information about the disabled person;
  • Contacts;
  • Medical Conditions;
  • Work Activity;
  • Education and Training;
  • Job History;
  • Medicines;
  • Medical Treatment;
  • Other Medical Information;
  • Vocational Rehabilitation, Employment or Other Support Services; and
  • Remarks.

The first 2 sections, Information and Contacts, gather the basic contact information for the disabled person and the person helping them complete the application (if any) or another person who could answer questions related to their disability status.

The Medical Conditions, Medicines, Medical Treatment, and Other Medical Information sections all gather extensive information about treatment history, testing, medications, and treating/prescribing medical professionals. These sections help identify the disabling condition and form a timeframe for the disability duration and severity.

The Work Activity, Education and Training and Job History sections serve to prove that the claimant worked enough years to qualify for Social Security disability benefits. It also serves to prove that they are no longer able to work because of their disability and the nature of their education, training and former employment.

While filling out a few sections on a form may sound simple, you will need to gather a good amount of records to be able to properly complete the Adult Disability Report. An incomplete report may delay or deny your Social Security disability benefits claim. To maximize your chances at a problem-free application process, you should contact a New York Social Security disability lawyer to make sure you have a valid disability case.
