
The New York Times is reporting that Qualitest Pharmaceuticals Inc. is recalling on of their products due to a defective insulin needle.
The product being recalled is the Accusure Insulin Syringe. Qualitest Pharmaceuticals Inc. has received three reports, so far, of the syringe needles sticking in the skin after the insulin was administered, becoming stuck inside of the insulin vial and sliding back into the syringe during use.
The syringes being recalled are the Accusure Insulin Syringes (1/2 cc - 31G - Short Needle) with the lot numbers 6JCB1 or NDC 0603-7001-21 (distributed to retail pharmacies and wholesalers in the US and Puerto Rico between January and June 2007) and Accusure Insulin Syringes (1 cc - 31G - Short Needle) with the lot numbers 7CPT1 or NDC 0603-7002-21 (distrubuted to retail pharmacies and wholesalers in the US and Puerto Rico between May 2007 and June 2008). The lot number can be found on the white paper backing of each individual syringe.
Consumers with the recalled product are urged to stop use of the Accusure Insulin Syringe immediately to avoid risk of injury. For product replacement instructions contact Qualitest Pharmaceuticals Inc. at 800.444.4011.
